Anyone can report — a participant, a family member, a support worker, a coordinator, or a member of the public. You do not need an account with us, and you can report anonymously.

We take every report seriously. As a registered NDIS provider we are required to record, investigate and — where the incident meets the threshold — notify the NDIS Quality and Safeguards Commission. You do not have to work out whether it meets that threshold. Tell us what happened and we will handle it.
You can also report directly to the NDIS Commission at any time, instead of or as well as telling us: ndiscommission.gov.au/about/complaints or 1800 035 544. You never need our permission to do that, and we will never discourage it.
Fill in what you can. Nothing except the description is required — a partial report is far better than no report.
If you have given us a way to contact you, we acknowledge your report within one business day.
The report is recorded in our incident register with a severity assessment, and checked against the NDIS reportable-incident criteria.
Where an incident is notifiable, we report it to the NDIS Commission within the required timeframe — 24 hours or 5 days depending on the category.
We investigate, take action, and — if you have given us contact details — tell you the outcome. Reporting in good faith never disadvantages you.

If you would rather not contact us at all, that is completely fine. The NDIS Commission takes reports about any provider directly, and an interpreter can be arranged free of charge.
